Acceptance of your order:
Once you have entered your payment and personal details and placed your order, we shall send you an email or a message acknowledging the receipt of your order and an invoice for your order. Your order will be accepted and the order will be completed when we send you an email or message to confirm the goods have been dispatched to you.

Delivery options:
1. Order and Collect
Please bring your cheque / pay-in slip together with this form to Community Health Organisation for Intervention, Care and Empowerment Limited, Rm1001A, Witty Commercial Building, 1A-1L Tung Choi Street, Mongkok, Kowloon, Hong Kong or email to info@mylife-choice.org.
2. Local courier service
For orders in Hong Kong, we use local courier service as our delivery company. Below is the summary of delivery costs per order:

Please settle your order payment at least one day before delivery of product. If you have selected to bank transfer, please also send us the transaction script that help us to guarantee your product will arrive on-time.

Note :
Above delivery cost is applied to industrial and commercial area in Hong Kong ONLY, for neither industrial nor commercial area, or remote locations, additional delivery costs will be charged by logistics company upon the confirmation of recipient address.

If your delivery address is out of Hong Kong, we will send over the parcel through Hongkong Post, delivery cost to be borne by customer.

If customer requests a specific date of delivery, we will convey the request to local courier service. However please note that as the delivery involves the handling of the courier company, the actual delivery date and time is not guaranteed.

If a product is out of stock or requires longer delivery time, we will state clearly on the product page and will notify our customers immediately. Refund due to out of stock will be processed and completed within 7 working days. However, delivery time may be subjected to logistics arrangement, weather conditions, public holidays and etc. We do not accept liability for any late delivery due to the above reasons.

If the reasons for delivery failure includes wrong or incomplete delivery address, or the recipient cannot be reached, the goods will be returned to C.H.O.I.C.E. center. We will try to contact you by email or phone, and arrange a new delivery. For re-delivery to commercial and industrial area, HKD30 delivery fee will be charged. If customer requests to cancel the paid orders, delivery charges are non-refundable.
